Job Details
Facilitates Quality Improvement Program and assists with care management initiatives. Attends workshops and training relevant to the position. Applicants must be a graduate of an accredited LPN program, current WV license, Board Certification in the appropriate area(s), and knowledge of professional medical theory, practice and care to administer and evaluate patient care.
Quality Improvement Responsibilities:

Performs and assists with audit reports as needed

Revises individual report formats as needed based on Committee recommendations

Reports QI data trends to care teams, Chief Medical Officer, Associate Medical Director, and Operations/Nursing Officer for further analysis and planning

Manages PDCA (Plan, Do, Check, Act) process with care teams and reports outcomes to the Clinical Provider Group and the QI Committee

Will work floor in the capacity as a licensed nurse to work with care teams reviewing work flow and systems to improve quality.
Operational Responsibilities:

Attends monthly NRHA Operational or clinical meetings as directed by CMO

Assists with training and staff development as requested

Reviews electronic medical records as requested by the Chief Medical Officer, such as provider organizers, providers’ schedules, past medical histories and labs

Assists the CMO with developing, updating and distributing clinical instruction and implementation of QI initiatives
Care Management Responsibilities:

Participates in NRHA care management initiatives
-
Monitors quality indicators required for each care management program and reports trends to the QI Committee

Oversees an assigned list of Care Management patients
